Overview
Social Worker – Behavioral Health in Tuba City, Arizona. This travel assignment supports both outpatient and inpatient social work services within a surgical center setting. The 13-week engagement starts on 09/07/2026 with a weekly pay range of $2,468–$2,493 and a 5x8-day schedule (Mon–Fri, 8:00 a.m.–5:00 p.m.). Responsibilities
- Provide outpatient and inpatient social work services to identified high-risk patients; assess needs, develop care plans, and coordinate services
- Collaborate with a multidisciplinary team (physicians, nurses, case managers, care coordinators) to ensure a seamless continuum of care
- Conduct discharge planning, utilization review, and assist with admission decisions for ED presentations and other hospital events
- Link patients with community resources, behavioral health and substance use services, and follow through to achieve measurable outcomes
- Monitor patient progress, reassess care plans, and advocate for patient and family engagement to promote safety and self-management
- Document care processes and utilize the electronic health record to support case management and care coordination across settings

Requirements
- Master’s degree in Social Work (MSW)
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with an active, unrestricted license in any U.S. state or territory
- Current Basic Life Support (BLS) certification from the American Heart Association
- Minimum three (3) years of clinical care experience; experience with discharge planning, case management, or utilization review
- Knowledge of electronic health record systems; ability to work effectively in inpatient and outpatient settings
- Ability to collaborate with a multidisciplinary team and work within a health system

Note: Full compliance is required two weeks prior to the start date.
